Consultation Fee (No Program): $150.00

Program Enrollment Fee: $875.00 (includes medical history, physical exam, medication review, baseline bloodwork and analysis, consultation with surgical team, follow up summary of results with clinic recommendations).
Post Procedure Follow up: $175.00/visit

Prescription Costs: Costs of prescriptions are extra and price varies based on your insurance coverage etc.
